The maximum dose of intravenous acetaminophen depends on both the patient’s age and weight. 

  • Pediatrics (< 2 years of age): The safety and efficacy have not been established.
  • Children (2-12 years of age): 12.5 mg/kg every 4 hours or 15 mg/kg every 6 hours, up to 75 mg/kg in 24 hours.
  • Adults and adolescents ≤50 kg: 12.5 mg/kg every 4 hours or 15 mg/kg every 6 hours, with a maximum of 75 mg/kg (up to 3,750 mg) in 24 hours.
  • Adults and adolescents ≥50 kg (no additional factor for hepatotoxicity): 650 mg every 4 hours or 1,000 mg every 6 hours, with a maximum of 4,000 mg in 24 hours.

Acetaminophen Injection is contraindicated in 7:

  • Patients who have previously demonstrated hypersensitivity to acetaminophen, to any ingredient in the formulation, including any non-medicinal ingredient, or component of the container
  • Patients with severe hepatic impairment or severe active liver disease

Indications and Usage

Acetaminophen for injection is indicated for the short-term management and treatment of 7:

  • Mild to moderate pain
  • Moderate to severe pain with adjunctive opioid analgesics
  • Fever

Pediatrics (< 2 years of age) 7:

  • Based on the data submitted and reviewed by Health Canada, the safety and efficacy of Acetaminophen for Injection in pediatric patients < 2 years of age has not been established; therefore, Health Canada has not authorized an indication for this age group.
